Case Studies - RVU Clicker - Medical Software

Client Requirement
The “RVU” (Relative Value Unit) system is a system employed by the Federal Government to determine payment for physician services. The client required that the system should define different medical procedures/services by assigning a weighted value to each service so that they could be directly compared in terms of work effort required to perform the procedure/service, as well as the liability risk and other factors.
Challenges
A lot of time was allocated towards technical feasibility mapping and analyzing the requirements and providing the best suggestions for the following:
Technology
Database
Preparing and integrating an algorithm
Compare productivity of physicians to their colleagues even though the doctors may perform very different types of procedures,
Track the productivity of numerous different shifts when performed by one doctor and by many different doctors
Provide real-time feedback to the doctors so they are aware of how productive they are relative to their colleagues and established benchmarks
Provide real-time productivity information to our administrators and teleradiology center for the purpose of workload balancing
Translate value-added non-RVU work effort into a quantifiable (RVU) format.
System software Requirements
.NET 3.5 Framework
SQL Server 2005 or SQL Server 2008 database in Server system only
At least 250kbps speed of internet connectivity for accessing the centralized RVU Database
The installation system must have the operating system with Windows XP, Vista, Win 7, Win 2003 , Win2008 and other Windows OS supporting Microsoft .Net Framework
Minimum 25MB disk space for installation.
Features
SDI proposed RVU Clicker system to be divided in two parts.
Interface for the Physician which enables a user (physicians) to efficiently log cases and assess his productivity in real time (among other features)
Interface for an Administrator which permits the user to determine relative productivity in real-time across the practice and assist in workload balancing decisions.
Administrator’s Panel
Monitoring the physicians’ performance diagrammatically and textually
Manage required content for RVU
Common Messaging for Live Physicians
Database connection status and changing the database connection
User [Physicians & Admin] management
Logo management [Changing the Application Logo]
Links Scheduler: Launching URLs
Shift and Profile management.
Physician
Physicians can enter and modify their examinations, memos and notes
Managing personalized profiles – [Grouping the examinations]
Physicians can preview the entered examinations ,memos and notes
Physicians can diagrammatically see their performance against to their target.

Planning
With the prior experience in developing Windows / Web based applications, SDI followed a Multi-tire approach towards the development starting from:
The Database layer containing MS SQL Server Database, Tables, Stored procedures and so on.
The Data Access layer containing the libraries responsible for accessing data from database.
The User Interface Layer which forms the Graphical User Interfaces.
Architecture
Microsoft .Net and MS SQL Server formed the basis of development. It is a Client / Server Windows Application and multi-threaded windows application based Windows Timer Component.

The Data Access Layer contained the libraries responsible for accessing data from database and built a generic query for insert, update, delete and select, depending on the business class.

All business layer components were coded to use this library for data accessing process from database. The UI layer was kept free of any business logic with images, applications and data being called from server.

Features of Architecture:
Each client will be coordinating with server simultaneously
Same client can be working in different locations. The interaction and inputs happening in one location will reflect automatically in all other locations without any refreshment.
If same client (user) has logged in to more than one machine and any one of the same client (user) logs out, then all other clients (users) will be logged out automatically.
Data refreshment will happen automatically between clients [physician views] and server [Administrator].
Client Feedback

“Several of our doctors have been using it and it has been working well".

Thanks
Wayne
